Direct Support Worker (Mentor) – Positions in Hillsborough, NH and Epping, NH

• Support individuals through work experience on our farm, daily living activities, as well as other community activities or appointments
• Maintain accurate records which includes daily notes, medication administration, and various tracking logs
• Assist in implementing and monitoring each person’s Individual Service Agreement goals
• Maintain safety, appearance and cleanliness of program spaces

Direct Support Team Lead – Hillsborough, NH

• Collaborate with area agencies and guardians; develop a plan for each resident which includes goals development and modifications as necessary
• Continually monitor and evaluate resident’s progression through Individual Service Agreement (ISA), and develop revisions to the plan as needed. Link residents with support and services as specified by their ISA goals
• Maintain complete documentation of internal and external communications including with medical providers
• Develop and submit required statistical data for reporting purposes
• Observe and report on resident’s behavior, presentation, safety, hygiene, etc.

Direct Support Supervisor, 3^rd Shift – Hillsborough, NH

• Make regular rounds through each building during the overnight shift to ensure staff are monitoring and completing overnight check lists; document rounds on logs
• Participate in the training of overnight residential staff on house operations and rules; ensure that new staff attend all required trainings
• Provide supervision for Awake Direct Support staff and document all staff supervision meetings
• Complete necessary paperwork and/or ensure that staff members have completed appropriate paperwork including, but not limited to: overnight shift reports, incident reports, accident reports, shift logs, and staff evaluations
• Coordinate medications at residences for overnight shift 
• Provide Nurse Trainer with updates of acute medical concerns
• Create staff schedule for upcoming week
